Starting services should feel simple, supportive, and stress-free. Our team will guide you through each step of the process.
Reach out to our team by phone or referral. We welcome individuals, families, Support Coordinators, schools, and community partners seeking DDA services and supports.
We take time to learn about the individual’s goals, preferences, support needs, interests, and current services to better understand how we can provide meaningful support.
Our team works closely with the individual, family, and Support Coordinator to review DDA eligibility, assessments, Individualized Service Plans (ISPs), and any required documentation.
We develop a person-centered support plan focused on independence, community involvement, safety, skill development, and meaningful outcomes tailored to each individual.
Once approved, we coordinate a smooth transition into services while maintaining ongoing communication and support throughout the onboarding process.
Whether you are exploring services for yourself, a loved one, or someone you support, our team is here to guide you through the process with compassion, clarity, and care.
